This policy describes how OneForm (“we,” “us”) collects, uses, and shares personal information, including for purposes of the California Consumer Privacy Act (CCPA/CPRA).
Information we collect
- Contact form submissions. If you scan a QR code on a mailer and submit the contact form, we collect the name, email address, phone number, and contact preference you provide, along with the property address the mailer was sent to.
- Public records. Our permit database is compiled from publicly available building permit records published by city and county governments.
- Property information. We may associate public property records (such as parcel, ownership, and prior-sale information from public sources and licensed data providers) with addresses in our system.
- Usage data. Standard web logs (IP address, browser type, pages viewed) and QR-scan events (time of scan and the mailer’s tracking code).
How we use information
- To respond to your quote request and connect you with a licensed insurance producer or real-estate professional.
- To measure the performance of mail campaigns (e.g., which mailers were scanned).
- To operate, secure, and improve the service.
How we share information
- With the licensed producer or brokerage that sent your mailer, so they can follow up on your request.
- With service providers that host our infrastructure (e.g., cloud hosting) under contractual confidentiality obligations.
- We do not sell your personal information as defined by the CCPA.
Your California privacy rights
California residents may request: (1) disclosure of the personal information we hold about them, (2) deletion of that information, and (3) that we do not sell or share their personal information. To exercise these rights, email support@oneforminsurance.com. We will verify your request and respond within the time required by law. We do not discriminate against you for exercising your privacy rights.
Data retention
Lead submissions are retained while the associated account remains active or as needed to comply with legal obligations, and are deleted upon verified request.
Children
The service is not directed to children under 16 and we do not knowingly collect their personal information.
Changes
We will post any changes to this policy on this page with an updated date.
